We value you as an individual, and encourage your participation in decisions affecting your health care. A real person will always answer the phone, and you will almost always see the same doctor.

We want you to know that our medication recommendations for you are based on your needs and the best available clinical evidence, so we generally do not accept pharmaceutical promotional materials or free drug samples.

Because we value your time, we offer you the option of completing our new patient forms at your convenience. Simply fill out the forms below and bring them with you to your appointment.
(If you are under 15 years old, your parent or legal guardian will need to sign these in most cases)

Insurance

We are preferred providers for most major insurance panels. We recommend that you contact your insurance company to confirm your benefits. Copays and deductibles are due at the time of service as required by your agreement with your insurance company.

Medicare and Medicaid

Dr. Robinson has spent a significant portion of his professional career serving people who often find it difficult to obtain primary health care services such as migrant farmworkers, urban Indians, people without insurance and those who rely on state or federal health care plans. In keeping with his mission of service, we are currently accepting new Medicaid (Oregon Health Plan) patients with Marion Polk Community Health Plan, and new Medicare Advantage patients.

Cash Discount

For our clients without health insurance coverage, we offer at least a 20% cash discount for most services if you pay at the time of service. We also offer significantly discounted outside lab fees if you pay at the time the lab is drawn.
